ABSTRACT

Retrorectal hamartoma, also called tailgut cyst, is a congenital lesion resulting from the non-regression of embryo remnants of the hindgut. We describe in this work the case report of a 68-year-old man, previously healthy, complaining of rectal bulging for 4 months, which was diagnosed by nuclear magnetic resonance imaging as a multi-loculated and mucinous lesion, suggestive of tailgut cyst. In view of the finding, the lesion was surgically resected, due to the potential for future complications, and, through the anatomopathological analysis, there were no findings of malignancy in the specimen. ABSTRACT

Abstract Langerhans' cell histiocytosis is a rare disease characterized by proliferation of Langerhans cells in the body. It affects mainly males, predominantly in childhood. Ulcerated plaques are one of the cutaneous forms of presentation. Diagnostic confirmation is done through immunohistochemistry. As therapeutic options, topical corticosteroids and chemotherapy are good choices. The case is reported of a male patient, aged 14, with perianal ulceration. He consulted a coloproctologist, who performed a biopsy of the region and started local triamcinolone applications. Immunohistochemistry diagnosed Langerhans' cells histiocytosis. Further investigation revealed diabetes insipidus, osteolytic lesions in the skull and lower limbs, enlarged liver, and encephalic alterations. Chemotherapy was started with Vinblastine, with significant improvement of the lesions.


Resumo A histiocitose de células de Langerhans é uma doença rara caracterizada pela proliferação de células de Langerhans no corpo. A doença afeta principalmente os homens, predominantemente na infância. Placas ulceradas são uma das formas cutâneas de apresentação. A confirmação diagnóstica é feita através de análise imuno-histoquímica. Como opções terapêuticas, corticosteroides tópicos e quimioterapia são boas escolhas. O caso aqui relatado é de um paciente do sexo masculino, com idade de 14 anos, com ulceração perianal. Ele consultou um coloproctologista, que realizou uma biópsia da região e iniciou o tratamento com aplicações locais de triancinolona. A análise imunohistoquímica diagnosticou histiocitose de células de Langerhans. Outros exames revelaram diabetes insipidus, lesões osteolíticas no crânio e nos membros inferiores, aumento do fígado e alterações encefálicas. A quimioterapia foi iniciada com vimblastina, com melhora significativa das lesões.

ABSTRACT

Colorectal cancer is a common malignancy in our clinical practice and it often evolves to a metastatic stage. Cutaneous dissemination, however, is a rare form of presentation of this disease. This study reports the case of a 38-year-old female patient that, even after neoadjuvant chemotherapy, presented cutaneous metastases of signet-ring cell colorectal adenocarcinoma. The malignancy proved to be extremely aggressive, without response to clinical therapy nor allowing surgical management, leading the patient to death about six months after the diagnosis. (AU)


O câncer colorretal é uma neoplasia comum em nossa prática clínica e frequentemente evolui ao estágio metastático. Disseminação cutânea, entretanto, é uma forma rara de manifestação da doença. Apresentaremos aqui o caso de uma paciente de 38 anos que, mesmo após a realização de quimioterapia neoadjuvante, apresentou metástases cutâneas de um adenocarcinoma colorretal com células em anel de sinete. A neoplasia demonstrou-se extremamente agressiva, não respondendo ao tratamento medicamentoso nem permitindo o tratamento cirúrgico, levando a paciente ao óbito cerca de seis meses após o diagnóstico. ABSTRACT

Os autores descrevem um caso de massa abdominal epigástrica palpável associada a sintomas dispépticos em uma paciente jovem sem antecedentes patológicos. Estudos por imagem evidenciaram corpo estranho gástrico de "aspecto enovelado". A paciente foi submetida a gastrotomia via laparotomia, com retirada de um tricobezoar de 1.950 gramas.


The authors describe the case of a palpable epigastric mass associated with dyspeptic symptoms in a young female patient with no previous pathologic history. Imaging studies demonstrated the presence of a gastric foreign body with an "entangled appearance". The patient underwent laparotomy and gastrotomy for removal of a trichobezoar weighing 1,950 grams.
